He really means it when he says he didn't have a choice. Special Counseling is when the hospital takes you aside and brainwashes convinces you to do things you wouldn't normally do. Do you understood?

Not every time, but almost. That's why if you ever meet another prisoner who seems strangely aggressive or wants you to go back to your room, you should be careful. They might be following somebody else's orders.
